In water of mars when the nuclear explosion went off, what if the Doctor was in that and completely vapourised. How would he regenerate? Where would he appear?

He might not appear at all, if there was nothing left to regenerate...but nothing is really settled. In "The War Games" episode 10 (1969), the Doctor (Patrick Troughton in his final regular appearance as the Doctor) says that the Time Lords can live forever, barring accidents.
In Robert Holmes' "The Deadly Assassin" (1976), the thirteen lives rule is introduced... and there are instances of permanent Time Lord death from shooting in both stories.
What I want to know is how the Doctor managed to get his clothes to regenerate with him with the changes from selves 1-2 and 4-5!

...although some of the "Time Lords" shown on screen may not actually have been Time Lords....
...but lots of things that happen on Doctor Who can easily be covered in a line. For instance, Russell T Davies said that he nearly included a line along the lines of "thank goodness we were given 152 extra regenerations to fight in the Time War", thus ensuring many future Doctors.
What if Tom had survived the fall off that tower, and sacked the 3 assistants straight after...

Tom didn't fall off the tower. The Fourth Doctor fell from the tower. Tom Baker wasn't responsible for hiring or firing the performers playing the Doctor's "assistants". If Tom had fallen from the tower, he really wouldn't have been in much of a state to sack anyone because he would have required medical attention. Fortunately, as Tom was leaving the show anyway and the assistants were all set to accompany the new Doctor, hopefully Tom's injuries wouldn't have drastically affected production....and as Tom spent a lengthy period of being out of work following his stint on the show (unlike Patrick Troughton obviously, who was back at the BBC playing the Duke of Norfolk opposite Keith Michell's Henry VIII a couple of weeks after he finished on Doctor Who), he would have had plenty of time to fully recuperate at length without it harming his career.
The Mark Gatiss-scripted episode, "The Idiot's Lantern" had included a line where the Doctor said that he didn't like towers because he fell off one once but it was removed at a late stage.

Talking of Rainbow, the latest DWM has a picture of Zippy (voiced by Doctor Who actor & voice artist, Roy Skelton) influenced by "Time Lord victorious", noting that it has led to the death of Bungle.
